  • By car via Huntington Drive and Fair Oaks (about 10 to 12 minutes)Take Huntington Dr west toward South Pasadena and turn right on Fair Oaks Ave heading north, then turn right on Glenarm St, then left onto Arroyo Pkwy. A photo ID, your insurance card if you have one, and a list of any medications and allergies. For physicals, bring the form your school, employer, or surgeon needs signed.
